Requirements
1. Active Registered Nurse (RN) license in the state of residence.
2. Minimum of 2 years of clinical nursing experience.
3. Experience in telehealth or remote patient monitoring preferred.
4. Proficiency in using telehealth platforms and electronic health records (EHR).
5. Excellent communication and interpersonal skills.
6. Strong organizational and time management abilities.
7. Ability to work independently and as part of a team.
Responsibilities
1. Conduct virtual patient assessments and triage based on established protocols.
2. Develop and implement patient care plans, in collaboration with other healthcare providers.
3. Monitor patient progress and adjust care plans as necessary.
4. Provide health education and counseling to patients and their families.
5. Document patient interactions and care plans accurately in the EHR system.
6. Participate in ongoing training and professional development activities.
7. Maintain patient confidentiality and adhere to all regulatory and compliance standards.
