Requirements
Active RN license in the state of residence
Bachelor’s degree in Nursing (BSN) preferred
Minimum of 3 years of clinical experience
Previous experience in case management or consulting roles is a plus
Proficient in using electronic health records (EHR) systems
Strong analytical and problem-solving skills
Excellent verbal and written communication skills
Responsibilities
Provide remote consultation and clinical guidance to patients and healthcare providers
Conduct comprehensive health assessments and develop care plans
Coordinate and manage patient care, ensuring continuity and quality
Collaborate with interdisciplinary teams to optimize patient outcomes
Utilize telehealth platforms to deliver services and monitor patient progress
Educate patients and families on health management and treatment options
Maintain accurate and up-to-date patient records in compliance with regulatory standards
