Schedule: Monday - Friday, 8:00 AM - 5:00 PM (CST/EST)

Pay Rate:- $ 17 / hr On W2

Job Responsibilities
The Medicare Population Health HRA Care Navigator will:
?onduct telephonic outreach to Cigna Healthcare Medicare customers to complete health screenings.
?ducate customers on plan benefits and address gaps in care.
?oordinate with internal teams to ensure timely hand-off to appropriate care teams.
?ocument all interactions for regulatory review.
?ssist with scheduling medical appointments and connecting customers to case management and community resources.
?scalate customer concerns or issues appropriately.
Required Skills & Qualifications
?trong written and verbal communication skills with an empathetic attitude.
?roficiency in navigating computer applications, including Microsoft Outlook, Excel, Word, and dual-monitor setups.
?bility to handle multiple tasks, set priorities, and develop action plans with attention to detail.
?nowledge of healthcare and regulatory requirements with an emphasis on Medicare.
?xperience in managed care, health services, or population management is preferred.
Education:
?igh school diploma required; college degree preferred.
Experience:
?+ years in managed care or a related healthcare field.
?+ years involving telephone contact and process management preferred.
